Statement: Cree Canada is committed to the well-being of our customers and has decided to initiate a voluntary recall of a number of LED T8 replacement lamps. While known incidents are very small in relation to the number of products shipped, any potential safety issue is unacceptable.

Cree Canada is working with the Consumer Product Safety Commission, Health Canada and Profeco in Mexico to provide information to Cree® LED T8 Replacement Lamp owners.

1. Hazard: Electrical resistance between the spring contact and the printed circuit board (PCB) may cause over-heating due to electrical arcing. The printed circuit board can overheat and cause the LED T8 lamp to melt, posing fire and burn hazards.

2. Affected Units:  ~ About 700,000 lamps in the U.S. In addition, Cree Canada has about 11,500 lamps affected.

3. Description: This voluntary recall involves the Cree® LED T8 Replacement Lamp. Product numbers, package UPC and date codes for affected lamps are listed below. The lamps are white in color and have “BT848 Series Lamp” followed by the product part number on the line beneath, printed on a white label or directly on the tube at one end of the lamp. A four digit date code is printed beneath the ballast compatibility statement “Compatible with Instant Start, Rapid Start and Dimmable Electronic Ballasts.” The lamps give off white light and are used indoors to replace traditional T8 fluorescent lamps.

4. Remedy: Customers should immediately stop using the affected LED T8 lamps, disconnect or switch off the fixture and remove the Cree® LED T8 Replacement Lamp. 5. Sold at: Sold to commercial lighting customers through electrical lighting distributors, from October 2014 and May 2015.

What problem has Cree identified with its LED T8 Replacement Lamps?

Can I elect to get a refund?

Yes, you may elect to receive a refund of the full amount you paid for the lamps.

Is the exchange program free of charge?

Yes. If you own one or more affected Cree® LED T8 lamps, you will receive free replacement product. Alternatively, you may elect to receive a refund of the full amount you paid for the lamps. Customers will be required to return the LED T8 lamp. Cree Canada will pay for return shipping costs and will provide appropriate packaging. A return shipping label will be included in the packet Cree Canada will send you once you have registered for the recall.

Do I need a receipt?

Commercial and institutional installation customers will need to produce an invoice or bill of sale in order to register for the free refund or replacement. Cree Canada will pay for Commercial and institutional installation customers return shipping costs and will provide appropriate packaging. For residential consumers, a cash register receipt or invoice would be appreciated when you contact Cree Canada, but is not necessary. However, residential consumers without a receipt will be required to return the LED T8 lamps as proof of purchase. Cree Canada will pay for residential consumers’ return shipping costs and will provide appropriate packaging.

1. Hazard: The recalled lamps can overheat and melt, posing a burn hazard.

2. Affected Units: 8,500 were sold in Canada (in addition About 104,000 in the U.S.)

3. Description: This recall involves Cree LED T8 Replacement Lamps, including lamps that were provided as free replacements for a previous recall in June 2015 and new lamps sold since the recall. These lamps are used indoors to replace traditional two-pin T8 fluorescent tubes. The white lamps have a cylindrical shape and measure 48 inches long. “Cree,” “BT848 Series Lamp,” the product number and a four-digit date code are printed on the lamp or on a white label affixed to the lamp.

4. Remedy: Refund

Consumers should immediately stop using, disconnect or switch off the fixture, remove the recalled lamp, put it in a safe place and contact Cree to receive a full refund. If consumers have received a free replacement lamp as part of the previous 2015 recall, they also should stop using the replacement lamps and contact Cree.

6. Sold at: Home Depot stores nationwide from September 2015 through May 2016 for about $29 per tube. This recall includes LED T8 lamps consumers received as free replacements from June 2015 through May 2016 for the previous recall.
